Gather essential and accurate information about the patient.
- Observe and competently perform a palliative care assessment on a patient with advanced disease with common symptoms and concerns, including physical, psychological, social, and spiritual experience and needs of patient and family.
Make informed decisions about diagnostic and therapeutic interventions based on patient information and preferences, up-to-date scientific evidence, and clinical judgment.
- Perform daily assessments of changing patient needs related to physical symptoms and psychological, social, and spiritual needs.
- Demonstrate awareness of and respect for patients' needs for symptom relief, control, and dignity.
- Estimate prognosis for patients with cancer, stroke/neurologic disease, dementia, COPD, CHF, ASCVD, ESRD, HIV.
- Understand pharmacology: opioids, non-opioid analgesics, anxiolytics, antidepressants, antipsychotics, anticholinergics, cathartics, steroids, psychostimulants, antiemetics.
Develop and carry out patient management plans.
- Formulate of initial management plan involving multiple disciplines in care, with attention to physical, psychological, social, and spiritual needs of patient and family.
- Participate actively in interdisciplinary team rounds through attendance, presenting patients, offering opinions and information, and suggesting strategies and interventions.
Counsel and educate patients and their families regarding:
- Opioids and pain management
- Advance directives and DNR orders
- Use of artificial nutrition and hydration
- Withdrawal of life support
- Requests for hastening death
- Symptom management (including constipation, nausea/vomiting, diarrhea, dyspnea, depression/anxiety, agitation/delirium, fatigue, anorexia, generalized weakness, oropharyngeal problems)
- Anticipatory guidance regarding life completion and closure for terminally ill patients and their families
Use information technology to support patient care decisions and patient education.
- Research medical literature pertinent to patient care and provide education for other team members.
