- Discuss the role of a physician as part of a palliative care interdisciplinary team.
- Describe the roles of each member on a palliative care interdisciplinary team.
- Acknowledge the contributions of other disciplines to patient care; identify resources to assist patients and families in the hospital and in their communities.
- Discuss the role of hospice care in patient-centered care.
- Discuss approaches to integrating community agencies and services within a comprehensive patient-centered plan of care.
- Identify local community resources for home health care.
- Collaborate with Clinical Resource Coordinator (CRC) to complete a hospice/VNA referral.
- Identify bereavement resources for grieving families.
